Germany is short of care staff. Homes, hospitals and outpatient services hire from abroad, and official portals still list tens of thousands of vacant care posts. Visa sponsorship here means a real German employer, recognition of your qualification, and the correct residence permit — not a bought “guarantee.”
Pay is in euros. The range in the care sector in 2026 runs from helper wages on the floor to six-figure packages at the top of large providers: senior Pflegedienstleitung, regional care directors, and medical directors. That is where €115,000+ can appear. It is not the starting salary for a care assistant.

TVöD-P from May 2026 puts a Pflegefachkraft around €3,510–€4,490 gross a month (P7–P8) before night, Sunday and holiday premiums and the annual bonus. Leadership grades go to about €6,900 a month at P16 Stufe 6 — plus allowances. Private chains and medical-director contracts sit above that table. That is the only honest path to €115,000+.
Sector minimum wage from 1 July 2026: helpers €16.52/hour, qualified helpers €17.80, qualified nurses €21.03. That is the floor, not the ceiling.
Visa routes for care homes
The EU Blue Card is not the usual nurse route. Most people use:
- §18a skilled worker — full recognition as Pflegefachkraft + qualified job offer.
- §16d recognition visa — partial recognition; you finish the exam or adaptation course in Germany and often work as an assistant meanwhile.
- Recognition partnership — enter with a contract and complete recognition on the job (often A2 German to start).
- Assistant track (§22a BeschV) — for people recognised as nursing assistants, with BA approval.
- Opportunity Card — job-search entry if you do not have an offer yet.
Employers often run the §81a fast-track so the foreigners authority coordinates recognition, BA approval and the embassy date.

Recognition and language
Germany compares your diploma with Pflegefachperson. Full equivalence lets you work as a nurse. Partial equivalence means a knowledge exam or an adaptation placement. Start at Anerkennung in Deutschland.
Language: A2 can be enough to enter on a recognition route. B1–B2 plus, in many Länder, a professional language exam is what you need to practise as a Pflegefachkraft. Care homes run in German.

- Group / medical directors in large providers: that is where €115,000+ is realistic.
Compare net pay minus rent, not the headline. Tax, social insurance and city housing change the number that reaches your account.
